Blackstone Valley Community Health Care is a patient centered medical home, with six locations (and growing!). Formed in 1990 by the joining of smaller health clinics in Pawtucket and Central Falls, Rhode Island, BVCHC has since expanded services and practice sites, including a separate dental facility, and a growing optometry clinic in downtown Pawtucket.

Located in an urban, culturally diverse setting, we serve over 18,000 residents of all ages. Our services include perinatal and women’s health care, pediatric, adolescent and adult preventative, acute care, behavioral health, optometry, and dental services, including oral surgery.

Blackstone is comprised of over 140 employees and 20 clinicians of all specialties. Most of our employees are multilingual in Spanish, Portuguese, and/or Creole. In addition, we stand apart because of our commitment to maintaining state of the art health information technology and electronic medical and dental records!
