Working as part of the care team, the Call Center Representative is responsible for providing outstanding customer service and will be the liaison between the organization and the community including current and potential patients, community partners, and vendors. It is the responsibility of this position to accept ownership for effectively addressing patient issues, complaints, and inquiries while ensuring customer satisfaction is at the core of every decision and displayed behavior.
The Dental Hygienist is responsible for providing outstanding patient care and performs dental measures designed to preserve teeth and prevent the spread of oral disease. The Dental Hygienist displays responsible behaviors, communicates effectively to others and functions as a member of dental team. The Dental Hygienist must demonstrate competency in all skills related to the performance of patient care.
The Dental Secretary is the first point of contact for visitors/patients entering the clinic. It is the responsibility of the Dental Secretary to greet visitors/patients, and triage and assess the reasons a patient is at the clinic. This position will work independently or with the care team, when appropriate, to provide excellent customer service and meet the needs of the patient.

Medical Assistants play an incredibly important role in the team-based, collaborative care that we provide through our clinics as they assist our providers in carrying out all actions and responsibilities in the care that we provide to our patients.
The lead facilitator of care coordination for high risk/cost patients. The NCM will utilize quality reports to focus the resources of the care team on providing additional support to achieve quality patient care management. This position coordinates clinical and case management for targeted patient assignments. Also, performs overall utilization management, resource management, care facilitation, and referral to other level of care. The NCM works with care teams and other resources including integrated and contracted behavioral health, CHT, and community resources to facilitate achievement of desire quality and financial outcomes.
Perform medical histories and physical examinations; assesses patients’ psychiatric conditions; administer treatments and in conformity with approved protocol; advise and counsel patients or legal guardian of patient problems; assist physician as directed, work with primary care provider to provide integrated care.
The Psychiatrist works as part of the care team to provide comprehensive psychiatric services as defined by Board granted clinical privileges. The Psychiatrist takes on a leadership role within the clinic as well as a supportive leader role for all other clinic support staff. The Psychiatrist will perform medical histories and physical examinations; assesses patients’ psychiatric conditions; administer treatments in conformity with approved protocol; advise and counsel patients or his/her legal guardian ; work with primary care providers and other behavioral health clinicians to provide integrated care.
The Registered Nurse works as part of the care team and is responsible for providing outstanding patient care in the primary care/medical home model environment. Working with clinicians, clinical support staff, and other staff members, the Registered Nurse ensures that patients are provided with high quality care in a timely manner, prompt response to phone calls, up to date with preventative health care maintenance protocols and understand their health care plan. The Registered Nurse functions under the direct supervision and authority of the Physician/Licensed Independent Practitioner when performing clinical tasks during patient care.
