The Health Coach Coordinator is responsible for overseeing the deployment of Community Health Worker Volunteers within Blackstone Valley Community Health Care. The Health Coach Coordinator collaborates with other staff and departments at Blackstone Valley to identify and resolve logistical challenges to implementing the CHW Volunteer program. The Health Coach Coordinator ensures the CHW Volunteer program follows prescribed policies, procedures, and protocols and meets outcome targets.
Provides comprehensive medical services as defined by Board granted clinical privileges.
Health Information Technicians maintain the integrity of all patient health information that is received from external/internal sources, respond to patient and facility requests for patient related information, and integrate received clinical information into the Electronic Health Record. Disclose health information in compliance with Rhode Island General Laws, Federal Public Health Laws and HIPAA Privacy Regulations. Assist the Manager and Director of Health Information in the various duties associated with Health Information. Monitors and ensures compliance, privacy, and information management aligns with overall organizational goals.
The Integrated Behavioral Health (IBH) Clinician provides direct clinical service to patients in an integrated team based model of care. Clinical service may include consultation/assessment (urgent basis or pre-scheduled), support and linkage services, psychoeducation, brief interventions, and counseling to enhance patient physical and psychological well-being. The IBH Clinician provides clinical supervision to behavioral health interns; participates in quality improvement and peer reviews; and assists in developing programming as requested by the Director of Behavioral Health Services.
The School Based Integrated Behavioral Health Clinician provides direct services to patients at the Blackstone Valley Community Health Center (BVCHC) Central Falls School Based Health Center (SBHC) including but not limited to individual, group, and crisis intervention modalities. The School Based Behavioral Health Clinician provides services as part of an integrated team.
The Medical Secretary is the first point of contact for visitors/patients entering the clinic. It is the responsibility of the Medical Secretary to greet visitors/patients, and triage and assess the reasons a patient is at the clinic. This position will work independently or with the care team, when appropriate, to provide excellent customer service and meet the needs of the patient.
The Psychiatrist works as part of the care team to provide comprehensive psychiatric services as defined by Board granted clinical privileges. The Psychiatrist takes on a leadership role within the clinic as well as a supportive leader role for all other clinic support staff. The Psychiatrist will perform medical histories and physical examinations; assesses patients’ psychiatric conditions; administer treatments in conformity with approved protocol; advise and counsel patients or his/her legal guardian ; work with primary care providers and other behavioral health clinicians to provide integrated care.
