The Integrated Behavioral Health (IBH) Psychologist provides direct clinical service to patients in an integrated team-based model of care. Clinical service may include consultation/assessment (urgent basis or pre-scheduled), support and linkage services, psychoeducation, brief interventions, and counseling to enhance patient physical and psychological well-being. The IBH Psychologist may provide clinical supervision to behavioral health interns and staff; participates in quality improvement and peer reviews; and assists in developing programming as requested by the Director of Behavioral Health Services.#1638

The lead facilitator of care coordination for high risk/cost patients. The NCM will utilize quality reports to focus the resources of the care team on providing additional support to achieve quality patient care management. This position coordinates clinical and case management for targeted patient assignments. Also, performs overall utilization management, resource management, care facilitation, and referral to other level of care. The NCM works with care teams and other resources including integrated and contracted behavioral health, CHT, and community resources to facilitate achievement of desire quality and financial outcomes. #1666

The PSR is responsible for effective management of the flow of in-bound calls; greeting and/or screening individuals; preparation and accurate registration of relevant patient appointments; assisting with population health outreach; accurate and efficient appointment scheduling, insurance plan verification; collection of payments; management of forms, insurance cards and patient photos and/or photo identification. In doing so, the PSR proactively and appropriately addresses issues in advance or acts timely to thwart potential workflow, patient satisfaction or safety related issues.#1669

The Registered Nurse works as part of the care team and is responsible for providing outstanding patient care in the primary care/medical home model environment to provide care collaboration with clinicians, clinical support staff, behavioral health providers, community health workers, and external case managers in the design and provision of care as part of the approach to population health management through a cohesive, multidisciplinary care team. The Registered Nurse ensures that patients are provided with high quality care in a timely manner, receive prompt responses to phone call and patient portal inquiries, and remain up to date with evidenced-based preventative health guidelines while promoting wellness and disease prevention in the community. The Registered Nurse also identifies patients who may benefit from outreach efforts to promote health screening and related primary care services. During periods of wellness and injury, the Registered Nurse provides triage per clinical protocols and directs patient towards appropriate level of care in collaboration with the provider develops and understands patients’ health care plan. The Registered Nurse functions under the direct supervision and authority of the Physician/Licensed Independent Practitioner when performing clinical tasks during patient care.#1635
