Under the direction of the Chief Medical Officer, the Clinical Director will provide leadership to the organization's clinical staff. This position will be instrumental in developing processes and structures that will impact the clinical teams and lead to higher patient and employee satisfaction. This position provides oversight for all operations related to clinical staff and patient services. This position assists the Chief Medical Officer in strategic planning and implementing clinical procedures and processes, and also provides supervision and coordination in all areas of clinical patient care (with the exception of dental).

The Community Health Worker is a member of the Community Health Team and acts as link to community-based organizations to facilitate patient access to health/social services.

Health Information Technicians maintain the integrity of all patient health information that is received from external/internal sources, respond to patient and facility requests for patient related information, and integrate received clinical information into the Electronic Health Record. Disclose health information in compliance with Rhode Island General Laws, Federal Public Health Laws and HIPAA Privacy Regulations. Assist the Manager and Director of Health Information in the various duties associated with Health Information. Monitors and ensures compliance, privacy, and information management aligns with overall organizational goals.

The Integrated Behavioral Health (IBH) Clinician provides direct clinical service to patients in an integrated team based model of care. Clinical service may include consultation/assessment (urgent basis or pre-scheduled), support and linkage services, psychoeducation, brief interventions, and counseling to enhance patient physical and psychological well-being. The IBH Clinician provides clinical supervision to behavioral health interns; participates in quality improvement and peer reviews; and assists in developing programming as requested by the Director of Behavioral Health Services.

The Medical Secretary is the first point of contact for visitors/patients entering the clinic. It is the responsibility of the Medical Secretary to greet visitors/patients, and triage and assess the reasons a patient is at the clinic. This position will work independently or with the care team, when appropriate, to provide excellent customer service and meet the needs of the patient.

The lead facilitator of care coordination for high risk/cost patients.  The NCM will utilize quality reports to focus the resources of the care team on providing additional support to achieve quality patient care management.  This position coordinates clinical and case management for targeted patient assignments.  Also, performs overall utilization management, resource management, care facilitation, and referral to other level of care.  The NCM works with care teams and other resources including integrated and contracted behavioral health, CHT, and community resources to facilitate achievement of desire quality and financial outcomes.

The Patient Flow Coordinator (PFC) is responsible for ensuring provision of outstanding patient care in the primary care/medical home model under a multidisciplinary team environment. The PFC effectively utilizes health information technology, administers timely care team support, and reinforce appropriate clinical documentation to enable other members to practice at the top of their license. The PFC displays responsible behaviors while demonstrating competency in all skills related to the performance of patient care.

The Quality Assurance Analyst is responsible for supporting practice transformation through process improvement and health information evaluation. This position will work within the Health Information Technology (HIT) department to identify, analyze, and execute initiatives to ensure best-practice processes and optimal HIT use throughout the clinic. This position will conduct business or project needs assessments, elevate the role of HIT in clinic sessions, support Electronic Medical Record (EMR) administration, and interpret initiative results to drive performance improvement through a variety of health information tools.
