New Employee Onboarding & Orientation
Welcome to BVCHC! Our Human Resources staff has provided you with the documentation HR and Occupational Health will need before you onboard.
*Your completed BCI must be received and reviewed prior to beginning employment.
*Please note: Our company policy prohibits employees from starting, or continuing to be seen, as a patient at any of our locations.
Human Resources
The following human resources forms and related documentation is optional to bring to your orientation. Please print out and complete the following forms, if you decide, and bring them to Human Resources on the day of your scheduled orientation. Click on each link to print forms:
- Federal W-4
- Rhode Island W-4
- I-9 Employment Eligibility Verification
- Affirmative Action/Veteran Self Identification
Documentation:
Please review the list below for examples of combinations of original documentation that can be provided to show proof of authorization to work in the United States (full list of acceptable documents found on page 3 of I-9 form):
- U.S Passport
- Valid Driver’s License & Social Security Card
- Valid Driver’s License & Birth Certificate (certified copy or original)
- Permanent Resident Card
Education, Licensure, Certifications (if applicable):
- Diploma or Certificate required for your position
- Professional license
- Certifications or Credentials (i.e. CPR, Medical Assistant Certification, etc.)
Occupational Health:
All of your immunization paperwork must be emailed to occhealth@BVCHC.ORG or by fax 401-728-1769 as soon as possible in order to prevent a delay in your start date. The Occupational Health Clinical Director, Kristen Lherisson, will review your records and let you know if/when you are cleared to start. Should you have any questions regarding your Occupational Health clearance, please contact Danielle Tremblay, Sr. Administrative Assistant at DTremblay@BVCHC.ORG or 401- 312-5248.
As per the RI Department of Health (RI DOH) regulations, you must receive Occupational Health clearance prior to beginning employment.
- Two step TB skin test (PPD). At your option, a BVCHC nurse can provide this test to you at no cost.
- Primary source verification or evidence of immunity (through titers) is required and should indicate Measles Mumps and Rubella (MMR) 2 doses, and Varicella vaccine (chicken pox) 2 doses. A record of current Tdap vaccine
- Influenza (flu) vaccine on or before December 1st of each year
- Hepatitis B vaccine series and/or titer. If not, this vaccine will be made available to you, at your option, without cost to you.
Additionally, if titers and/or labs are necessary, they can take 3-7 days for results. We require results by 1 week prior to your orientation.
Your Orientation Day
You will report to Human Resources located at 39 East Avenue, Pawtucket at for New Employee Orientation. During Orientation, you will be introduced to BVCHC, its history and services, go into greater depth regarding our Mission, Vision and Values, and our strong focus on providing the very best patient care experience. Our Human Resources department will inform you of the date and time for you to attend the Orientation. Please be aware that attendance at New Employee Orientation is a requirement for beginning your employment and completing your probation. If you cannot attend the scheduled Orientation, you will need to reschedule. Your supervisor will provide you with a job/department specific orientation beginning with your first day on the job. You will receive information from you supervisor which will identify elements of your department orientation.
Important Information for Current BVCHC Patients
Blackstone’s policy is that employees cannot be patients, even if the employee was a pre-existing patient. Any employees who were pre-existing patients will unfortunately need to cancel any appointments and find a new provider. If you have any concerns with this process from a clinical standpoint, please do not hesitate to contact our Risk Manager at 401-312-9838 for assistance.
